Understanding your staff’s actual wants and needs with respect to recognition will help you create personal initiatives. Personalized recognition will create a stronger tie between recognition and the desired behavior in the mind of the person being recognized. This survey template offers a series of sample questions that a CIO may wish to present to his/her staff to understand recognition preferences.

This survey:

  • Will help you identify the needs and wants of your staff.
  • Should be distributed to the entire department so that you can personalize initiatives for all the staff and/or produce initiatives that drive the majority of your staff.
