Requirements may change throughout the project life cycle. Ensure that the accurate assessment and impact of the change are documented whenever a signed off requirement is changed by recording the information. This form should be used by IT or the business unit when requirement changes are being requested. It tracks:
- The current requirement and the new requirement being requested.
- The rationale behind the request and the impact of the change.
Documenting and tracking these changes ensures accuracy when addressing the requests for change.
